Describe Non-Inflammatory Arthritis
Morning stiffness less than 30 minutes. Increases with activity, improves with rest. Worst at end of day. Locking/instability.
Describe Inflammatory Arthritis
Morning stiffness over 30 minutes. Worst at beginning of day. Improves with activity, worse with rest. No locking/instability.
What is Rheumatoid Arthritis and associated cytokines?
Chronic, systemic disorder involving synovial joints. Leads to destruction of joints. IL-6, IL-1, TNF. HLA-DR4 PTPN22 PADI 4 (arginine to citrulline)
RF and CCP?
RF = Rheumatoid factor proteins produced by immune system that attack healthy tissue. CCP = Anticyclic Citrullinated Peptide More SPECIFIC than RF.
What is found in RA imaging?
Joint space narrowing. Periarticular osteopenia Marginal erosions.
Describe Seronegative Spondyloarthritis
Affects axial skeleton. Large joints - shoulders, hips, knees. HLA-B27. Related to Ankylosing Spondylitis, IBD arthritis, Psoriatic arthritis, and Reactive arthritis. Seronegative- Negative RF.
Describe Sjogren's Syndrome
Dry eyes, mouth, dental caries, joint pain. SSA/SSB Antibody. Also ANA. Biopsy minor salivary gland see lymphocytic infiltrates. Drench night sweats and lymphoma. Treatment mostly supportive. Consider azathioprine, methotrexate, hydroxycholorquine in inflam. arthritis.
Describe Lupus
Young women and African-Americans. Rash, inflammatory joint pain, low grade fever. Autoantibodies lead to organ damage. dsDNA , Anti-Smith antibody. *Smith SPECIFIC. *Histone Ab is DRUG INDUCED LUPUS.
Describe osteoarthritis
Non-inflammatory degenerative arthritis. Joint narrowing Osteophytes Sclerosis & cytsts Stress alters metabolism. Chondrocytes activate. Cartilage breakdown results in synovitis and release of inflammatory cytokines. IL-1, IL-6, TNF. Adipose is proinflammatory. Decreased size, aggregation, and proteoglycans. Chondrocytes form clusters.
d
Cartilage damage resulting from the immune response to a bacterial element (Septic Arthritis) Proliferation of granulation tissue (Rheumatoid Arthritis) Localize immune response to a pathologic crystal (Gout)
What joints in hand are affected by OA?
PIP DIP CMC
Synovial fluid analysis of non-inflammatory arthritis?
Volume in knee high Clarity is transparent (normal) Color is yellow Viscosity high (normal) WBC increase Negative culture *Color clear is normal
Describe ANA testing
Highly sensitive for SLE but LOW SPECIFICITY (many without disease get false +).
Describe Limited Scleroderma
Skin tightening due to subcutaneous fibrosis. CREST Calcinosis Raynaud's Esophogeal dysmotility Scerodactyly Telangiectasias Autoantibody= Centromere.
Raynaud's SLE.
Dermatomyositis - Gottron papules. Cutaneous Lupus - Spares knuckles.
